Our Special Services: For prescription order(s) on wheelchairs, scooters, power chairs, walkers and beds we provide home visit services (Mandatory Under Most Insurance Guidelines ) - (Free of charge)- At the initial visit a Medical Device Consultant from Astar Healthcare will come out into your home to gather information, assist with completing AH paperwork, conduct in home safety use evaluation, complete a questionaire sheet, evaluate and document evidence of further client special needs, obtain RX ( CMN ) orders and answer any questions you may have prior to delivery of the medical device(s). If you have a Physician's RX ( CMN) and would like a special consulting agent to help fill out documents and conduct assessments at your residence, please call now to schedule. Prior to the visit Prescriptions and copies of two forms of ID's must be faxed to (916) 509-7050 ( Pre-visits required for certain insurance covered products)
